How to Record Vitals on a Client Chart
A simple, step-by-step guide. No computer experience needed.
Before you start
Log in to Olari with your own username and password. Have the client’s readings in front of you (from the machine or your manual check).
Who can do this: Administrator, Full No Admin, Full No Billing, and Detox Monitor. (Read Only staff can look at vitals but cannot add them.)
What “vitals” means: the client’s body measurements — such as blood pressure, pulse (heart rate), temperature, breathing rate, and oxygen level. A chart is the client’s own page (think client file) where their information lives.
PART 1: Open the Client’s Chart
- Go to the Client tab. From the dropdown menu, select one of the following:
- Census – Typically defaults to active client view, but you can switch to inactive if needed.
- List – View all clients, both active and inactive.
- To quickly find a client from either Census or List:
- Use the search bar located in the upper-left corner above the grid.
- Type the client’s name and press Enter.
- Click on the client’s name.
- The client chart will open.
- Use the tabs on the left side of screen to navigate locate “Vitals” tab and click it.
✓ What you should see: You see the client’s list of past vitals (it may be empty if none were taken yet).
- You will now be on the Vitals screen
- Click the button that says “Add” above the grid, to the far right.
- Add Vitals + Withdrawal Scale box will now appear on screen
- Check the date and time. It usually prefills with current date and time. Only change it if you are entering older readings.
- Type in Vitals into their assigned field — for example heart rate, blood pressure, oxygen, temperature, and any applicable notes.
⚠️ Validation Warnings: Olari checks your entries against expected clinical ranges. If a number is outside these limits, a warning message will appear asking you to review it before saving.

💡 Tip: If your facility also uses a withdrawal scale (such as CIWA or COWS), it is on the second tab in this screen. Fill it in the necessary values and then click Save.
✓ What you should see: The new reading appears at the top of the Vitals list with today’s date and time.
PART 3: Check or Fix a Reading
- If you did not take a certain reading, leave that box empty. Do not type a guess.
- When everything is entered, click the button that says “Save.”
- To check your work: look at the Vitals list. The newest reading is usually at the top.
- To fix a mistake:
- Click the Date Complete or the three dots (ellipsis) to the right of the appropriate Vitals record and select to Open
- Update the incorrect date, then click “Save” again.
✓ What you should see: The corrected reading now shows in the Vitals list.
If something goes wrong
- A red message appears when you click Save: A field may be empty when it is required, or it may have the wrong kind of value (like letters where a number goes). Read the message, fix that data, and click Save again.
- You cannot find an “Add” button near Vitals: Your role may be view-only. Ask your supervisor or the Olari administrator.
- You do not see a “Vitals” section at all: Your role may be restricted from viewing this information. Ask your supervisor or the Olari administrator.
- You are not sure it saved: Look in the Vitals list for your reading with the correct date and time. If it is there, it saved.
